Franciscan Community Counseling provides counseling and programming that promote emotional, mental, physical, and spiritual wellness for all people without regard to affiliation, orientation, or ability to pay.

It helps our staff to have all forms completed before your appointment. These options are available:

• Download PDF files from ‘Downloads’ list below, print them out, fill out the information and bring them to your appointment. To view the forms, click on the name of the desired form. The form will either be downloaded to your computer or it will display in a separate window depending upon file type, browser used, etc.
• Arrive 30 minutes before your appointment to fill forms out in our waiting room.
• Pick them up at our offices in advance of your appointment during normal hours and fill them out at your leisure.
